The five element fist routine contains the basic movements of chopping matches with metal (lung), crushing matches with wood (liver), drilling matches with water (kidney), exploding matches with fire (heart and pericardium), and crossing matches with earth (spleen). It actually addresses the six yin meridians and not five. How does Wu Xing Qi Gong address or differentiate the heart meridian and pericardium meridian?

The Heart and Pericardium are not differentiated in the wuxing qigong that I learned. Going back to the Huangdi Neijing, the Pericardium is like the minister, while the Heart is like the emperor. All contact with the emperor goes through his ministers, thus the Heart and the Pericardium are very closely related and, in general, working with one affects the other. In the case of the wuxing gigong, the movement of the Heart exercise has an action to squeeze the chest and massage the Heart. Since the Pericardium is wrapped around the Heart, it will certainly be massaged as well.

You can find the Pericardium Meridian in Huangdi Neijing, Ling Shu Jing, Jing Mai, verse 20.

Chapter 2 of the ling shu 《本輸》ben shu, lists the points for the channels. All of the points described for the Heart channel are on the Pericardium channel. No Heart channel points are described in the huangdi neijing, although they do appear in the zhenjiu jiayi jing which was published in the Three Kingdoms dynasty.

Great, this has confirmed the 11 meridians according to ancient text. It is interesting to note that Huangdi ask for 12 and got a reply of 11.

There is the heart meridian from verse 12 of Jingmai and there is the pericardium meridian from verse 20, and they run on different points.

From http://www.shen-nong.com:

The Heart Meridian starts from the heart, and divides into three branches. One goes towards the small intestine. The second runs upwards along the throat towards the eyes, and the third branch emerges under the arm and runs along the inner side of the forearm, elbow and upper arm. It then crosses the inner side of the wrist and palm and ends at the inside tip of the little finger, where it connects with the Small Intestine Meridian.

The Pericardium Meridian starts from the chest, leaves the pericardium organ and runs downwards through the diaphragm to connect with the triple burner. A branch rising from the chest emerges from the lower chest region and travels upwards to the axilla (armpit). From the medial aspect of the upper arm, it makes its way down between the lung and heart channels, until it reaches the elbow crease. It then runs down the forearm and enters the palm ending at the tip of the middle finger. Another short branch splits off from the palm to connect with the Triple Burner Meridian at the end of the ring finger.
